What to Expect from your Visit
What Do I Need to Bring With Me On My First Visit?
On each and every visit, please have your insurance card available so we can ensure your medical claims are filed correctly. You will be expected to pay all co-pay and deductible amounts due at the time of service. If you would like to know that amount prior to being seen at our facility, please contact your insurance company, and they can help you determine your benefit amount and any out-of-pocket expense you will incur.
Additionally, when scheduling your appointment, one of our central scheduling representatives will advise you of any/all clinical information you will need to bring for your visit.
Some of the items requested might include:
- Pertinent information about your medical history (including any prior surgeries or medical conditions)
- Any recent test results that may be pertinent to your injury/condition (i.e., X-rays, EMGs, MRIs, CT scans, etc.)
- A written list of medications you are currently taking (including prescription and non-prescription items, herbal medicines, supplements and vitamins)
- A referral form from your physician (if required by your insurance plan)
Where Do I Check In Once I Arrive For My Visit?
We appreciate the fact that you have chosen The Indiana Hand Center. In addition to providing the highest level of patient care, we want to make your visit with us is as smooth and pleasant as possible.
- If you’re visiting our main location at 8501 Harcourt Road, walk through the double doors to the right of the flag pole.
- Proceed through the double doors and you will be greeted by a patient services concierge located just inside the front doors. The concierge will provide you with all the necessary paperwork and instructions. You can then fill out your paperwork in the waiting room.
- Our patient services concierge will call you to the front desk area to review your paperwork. Once your information has been verified, your co-pay and/or deductible amounts have been paid and a copy of your insurance card has been obtained you will be asked to wait for the physician’s team to call you back to the clinic area.
Our patient services concierge staff will be here to greet you, review your paperwork, answer any questions you might have and provide you with the highest level of customer service.
