Please send $15.00 with the completed Authorization to Release of Medical Information Form. Any Additional charges will be billed to you at a later date. 4. Please allow 10—14 days for processing. Your insurance company representative can help you determine if a referral is needed and your family physician can generate the necessary paperwork to ensure your visit is pre-authorized. All referral arrangements should be completed before your first visit.
