FAQ

Question: How do I request a copy of my medical records?

1. Download/print the Authorization to Release Medical Information Form.
2. Complete the form in its entirety, including appropriate signatures.
3. Send the completed form and payment to:

Attn to: Release of Information

The cost should be calculated as follows:

• $20.00 for the first 10 pages and .50 for pages 11-50; pages 50+ are .25 per page.
  
• $6.00 for each x-ray

Please send $15.00 with the completed Authorization to Release of Medical Information Form. Any Additional charges will be billed to you at a later date.

4. Please allow 10—14 days for processing. Any questions regarding the release of your medical records should be directed to (317) 471-4476 (fax: 317-471-4471).

Question: What information will be included in the release of medical information?

Our standard release of information includes the following:

1. Physician’s dictated and hand-written notes
2. Therapist’s dictated notes
3. Copy of X-ray(s)

Question: How do I know if I need a referral for my upcoming visit?

You should refer to your primary care physician and insurance company for any needed referral information. Your insurance company representative can help you determine if a referral is needed and your family physician can generate the necessary paperwork to ensure your visit is pre-authorized. All referral arrangements should be completed before your first visit.
